  • Spanish and English versions are available, review the following section (on this page) for notes on the development of each version.
  • Body weight is a necessary part of scoring the calorie expenditure measures, thus, if you are collecting this as part of your study, please add it to the questionnaire.
  • You are welcome to modify the questionnaire if needed to meet the needs of a particular population (for example, add an activity that is common but not on this list). CHAMPS Physical Activity Questionnaire for Older Adults, 
English Version
This questionnaire is designed to be sensitive to changes in physical activity levels for primarily underactive older men and women. For example, it assesses the types and levels of physical activity that are meaningful and appropriate for older adults, including lighter as well as more vigorous activities. The questionnaire is designed to accommodate increasing sensory problems with age by using a large font, space on the page, and high contrast. However, Visually impaired individuals may prefer a personal or telephone interview or those with hearing impairments might do better with self-administration. The questionnaire completed by itself takes about 10-15 minutes on average, although this can vary from 5 to 20 minutes.

CHAMPS Physical Activity Questionnaire for Older Adults, 
Spanish Version
The CHAMPS Physical Activity Questionnaire was translated from English to Spanish using an iterative process (see "Translation Process" below). Please note that the Spanish version of the questionnaire has not been formally tested for reliability and validity. Staff estimated that it took participants approximately 20 minutes to complete the questionnaire on their own, or 20-30 minutes by interview.

Translation Process

The English version of the CHAMPS Physical Activity Questionnaire was initially translated, and back translated, by two individuals who were native Spanish speakers (one from Peru and one from Mexico), both with graduate degrees. The individuals met several times to discuss differences in wording and meaning. They also obtained input from a couple of other people of Mexican and South and Central American origin whose primarily languages were Spanish but did not have advanced educations. This latter step was critical because it was often difficult to determine the best wording to convey the meaning of the English phrases and still have the words understood by a broad-range of Latinos/Hispanics, especially since idiomatic Spanish varies widely between and across countries and social/economic levels. The goal of the translation was to have a version that would be clear to our target population of Spanish-speaking persons aged 50 and older in the Bay Area.
Upon completion of this initial translation process, there were multiple pilot tests of the physical activity questionnaire conducted in community populations similar to those who would be surveyed for the research program. As the pilot testing process evolved, further minor changes were made to the questionnaire instructions, formatting, and wording of survey questions. A few additional culturally relevant activities were added to both the Spanish and English questionnaires. Fairly often participants needed assistance to complete the questionnaire. The staff's impression was that the education and income levels of those in the pilot testing were lower than those in the prior CHAMPS projects that utilized the English questionnaire, although no objective measures were obtained to determine this. Difficulties often seemed related to either understanding the instructions, or having the math and/or reading skills needed to complete the questionnaire on one's own. The Spanish version of the questionnaire was pilot tested in primarily Mexican, South, and Central American populations.
