CHAMPS In Action

Three diverse community agencies in and around San Francisco, CA have adapted CHAMPS to meet needs of seniors in their communities. Each community has produced a unique version of CHAMPS, based on its participants' interests and available community resources. Read the descriptions below to learn more about these programs.

Practicing tai chi at Network for Elders

Seniors In Motion for Health (SIM)

  • Community: The Bayview/Hunters Point district of San Francisco, CA
  • Agency: Network for Elders (NFE)
  • Recruitment: Ongoing. Vehicles for recruitment include informational announcement at NFE monthly membership meetings, brochure, word of mouth, Media (print and radio).
  • Program Features:
    1. Physical activity resources (SIM and affiliated community classes: weekly exercise class at Joseph Lee Recreation Center, water exercise class at MLK pool, walking group, weekly community college exercise class, line dancing class)
    2. Newcomers folder
    3. Workshops on getting started, exercising safely,creating a balanced exercise program, stress management, nutrition, and memory
    4. Functional fitness testing
    5. Blood pressure monitoring weekly
    6. Support through telephone reminders, car pools, role models, special events (certificates of completion, picnics, field trips)

Exercising in the 30th Street Senior Center garden

Siempre Activo/Always Active

  • Community: The Mission district of San Francisco, CA
  • Agency: On Lok's 30th Street Senior Center
  • Recruitment: Ongoing. Recruitment occurs mostly through word of mouth or direct invitation. Visibility of program activities throughout the center also brings new participants into the program. Instructors of other classes (like nutrition and English) help promote Siempre Activo to their participants.
  • Program Features:
    1. All program meetings and materials are offered in Spanish and English.
    2. Workshops and talks including: monthly participatory workshops on exercising safely, flexibility, endurance, muscle strength, balance and fall prevention, stress management, memory; talks at special events increasing awareness of exercise
    3. Dr. Miller's health talks related to exercise (every other month). Topics include arthritis, diabetes, asthma, and insomnia.
    4. Materials including handouts on exercising safely, goal setting and activity diaries given at workshops, and calendar of events.
    5. Integration with other center activities (including tai-chi, yoga, dance) and health talks.

Group exercising at Sequoia

Aging with Energy

  • Community:The Sequoia Hospital Health & Wellness Services' Aging with Energy program is offered throughout the year at three community senior centers (Veterans Memorial Senior Center, San Carlos Senior Center, Belmont Senior Center). Sequoia staff go to the centers to offer the program.
  • Agency: Sequoia Hospital Health and Wellness Services
  • Recruitment:Each community senior center is individually responsible for recruitment. Recruitment methods vary across sites and include flyers, notices in newsletters, word of mouth, advertisement in Sequoia Hospital Community Calendar in local newspapers, and advertisement in the city's recreation guide. Peninsula Cable TV Channel provides a phone number for individuals to call regarding the program. Potential participants are invited to attend an informational meeting facilitated by Sequoia staff.
  • Program Features:
    1. Blood pressure
    2. Functional fitness testing
    3. Workshop series (Aging with Energy, Moving Forward: Using Your Functional Fitness Results (Part 1 and 2), Lifestyle Changes to a Healthier Heart, Balanced Eating for Life)
    4. Fall prevention class (lectures and exercises at Sequoia Hospital for all seniors, including those not physically able to begin Aging With Energy program; meets 1 hour per week for 4 weeks)
    5. In-home fall prevention and home safety program: This comprehensive program for seniors over the age of 65 is done on an individual basis and addresses all areas of fall prevention and home safety including exercise (especially balance exercises), medication use, and general health. This program can be done at Sequoia Hospital's Health & Wellness Services in Redwood City, at home, or by phone.
